Abstract

Saliva is a unique biological fluid that reflects the physiological state of both the oral cavity and the body as a whole, owing to the entry of biomarkers from the bloodstream via ultrafiltration and passive diffusion. The aim of this study was to evaluate the diagnostic and prognostic value of a specific panel of oral fluid biomarkers in the context of periodontal pathology and systemic diseases. The study involved patients with inflammatory periodontal diseases and metabolic syndrome, as well as a healthy control group. The methodology included the collection of mixed unstimulated saliva and the determination of levels of pro-inflammatory cytokines (IL-1β, IL-6, TNF-α), matrix metalloproteinases (MMP-8, MMP-9), and oxidative stress markers (malondialdehyde), alongside highly sensitive quantitative measurements of hormones and tumor markers using enzyme-linked immunosorbent assay (ELISA) and polymerase chain reaction (PCR). The results revealed a strong correlation between elevated salivary concentrations of MMP-8 and IL-1β and destructive processes in periodontal tissues, enabling the diagnosis of early-stage periodontitis prior to the onset of pronounced clinical symptoms. Salivary biomarkers demonstrated high diagnostic utility in assessing systemic inflammation associated with metabolic syndrome and carbohydrate metabolism disorders. The study concludes that salivary diagnostics represent a highly effective, non-invasive alternative to traditional blood-based testing methods. The findings underscore the potential for integrating oral fluid-based multiplex test systems into routine clinical practice for screening, monitoring disease progression, and assessing therapeutic efficacy.
